What Medicare actually pays for after a pacemaker or ICD

The device itself is Medicare-covered — that's the short answer. But it's covered under a completely different rulebook than the Durable Medical Equipment (DME) fee schedule this site tracks for every other checklist item on this hub. That split is the whole story: one big-ticket item with real Medicare coverage, and a small basket of retail items around it with none.

How is the device itself covered?

Two separate National Coverage Determinations, both live-fetched from CMS's Medicare Coverage Database this research pass:

• ICDs — NCD 20.4 (NCDId 110, version effective 07/31/2023). Benefit category: Prosthetic Devices. Verbatim: "CMS has determined that the evidence is sufficient to conclude that the use of ICDs, (also referred to as defibrillators) is reasonable and necessary" for specified indications — for example, a personal history of sustained ventricular tachyarrhythmia or cardiac arrest due to ventricular fibrillation, or a prior heart attack with reduced ejection fraction, subject to listed exclusions. • Pacemakers — NCD 20.8.3 (NCDId 357, version effective 08/13/2013). Benefit categories: Inpatient Hospital Services, Physicians' Services, Prosthetic Devices. Verbatim item description: "Permanent cardiac pacemakers refer to a group of self-contained, battery operated, implanted devices that send electrical stimulation to the heart through one or more implanted leads." Both are covered as prosthetic devices delivered during a covered procedure — closer, in Medicare's own structure, to how a hip or knee implant itself is billed than to how a walker or commode is billed.

Why isn't any of this in the DME dataset this site otherwise tracks?

Not a data gap — a scope difference, confirmed by reading all 68 codes in data/coverage_status.json directly. That dataset is entirely lower-extremity and general-mobility DME: canes, crutches, walkers and accessories, commodes, bath/toilet safety equipment, hospital beds and rails, patient-lift slings, seat-lift mechanisms, manual wheelchairs. There is no code for a pacemaker, an ICD, a cardiac lead, a seatbelt pillow, a bra, or a pill organizer, because none of them are Durable Medical Equipment in the first place.

Is a pacemaker or ICD in Medicare's own device classification registry?

No. data/dmecs_products.json — this site's copy of Palmetto GBA's DMECS product classification list (the same registry other hubs on this site cite for walker and toilet-seat HCPCS classifications, 9,806 products across 7 lower-extremity DME classifications) — has zero hits for "pacemaker," "ICD," "defibrillator," or "cardiac." That's consistent with the NCD finding above: this product category was never PDAC-classified as DME because it isn't DME.

What about recalls?

data/recalls.json has 2 raw hits for "pacemaker" — both are FDA convenience-kit recalls for hospital surgical-supply packs (procedure trays used during implantation itself, matched only because the recall pipeline's search term happened to also appear in a multi-item kit title alongside dozens of unrelated surgical trays). Read in full and confirmed not relevant to any consumer product on this hub's checklist. Zero hits for "ICD" or "defibrillator."
