How long is non-weight-bearing after bunion surgery? It depends on your technique.

There's no single answer — "bunion surgery" covers several different techniques, and how soon you can walk on your foot depends on which one your surgeon used. A traditional chevron osteotomy allows some weight the day after surgery. A traditional open Lapidus (joint-fusion) procedure requires a full cast and crutches for 8 weeks. Four independent sources (HSS, twice; AAOS; Cleveland Clinic) all confirm the same underlying pattern — weight-bearing depends on technique, not on "bunion surgery" as one thing. Confirm your own timeline with your surgeon; nothing here overrides their instructions.

The sharpest version: same hospital, same procedure name, two different timelines

The striking finding in this research isn't "minimally invasive is faster in general" — it's more precise than that. Hospital for Special Surgery publishes two separate patient-education pages that both describe the Lapidus procedure — a joint-fusion technique generally considered the most involved of the standard bunion surgeries. One page, medically reviewed by Jonathan T. Deland, MD, describes the traditional open version: a full cast and crutches for 8 weeks, no weight-bearing at all. A separate page, authored by Anne Holland Johnson, MD — a different HSS surgeon — describes the percutaneous/minimally invasive version of the identical procedure: weight-bearing allowed after just 2–4 weeks.

Same hospital. Same procedure name. A 4x-to-8x-shorter restriction, depending entirely on which approach was used — from two different HSS surgeons' own separate patient pages, not one surgeon describing two techniques. Dr. Johnson's own page names the mechanism directly:

“By avoiding large incisions, fewer soft tissues (skin, muscle, joint capsule, etc.) are disrupted, so the foot is less swollen and painful after surgery. This allows for immediate weightbearing, faster bone healing and a quicker return to baseline activity levels.”

A real, separate disagreement — not just about the timeline. AAOS, a broader orthopedic professional society (not an MIS-specialist practice), states on its own patient page: minimally invasive bunion surgery "has not been widely accepted, and more long-term research is needed before most foot and ankle surgeons will be willing to adopt this technique." That's a different claim than the weight-bearing numbers above — it's about how broadly the approach itself has caught on, not how fast any one patient recovers from it. Both views are real and independently sourced, as of each organization's own current page content. We present both rather than picking a side.

The honest gap: HSS's own page doesn't give a scarf number

HSS's bunion-surgery overview page states: "Those who have had a scarf procedure can bear partial weight at about weeks." The specific number is missing from that sentence — confirmed by checking both the page's rendered text and its own embedded structured-data markup, which repeats the identical gap. This is a real content gap on HSS's own live page, not a fetch error on our end, and we're not going to invent a number to fill it. If your surgeon used the scarf technique, ask them directly for your own timeline — the chevron (day 1) and Lapidus (8 weeks open, 2–4 weeks MIS) endpoints above are the closest bracketing figures HSS's own page does specify.

What this means for what you buy

The post-op surgical shoe, wedge pillow, and ice wrap on the main checklist apply no matter which technique you had — everyone ends up in the shoe eventually, once weight-bearing is cleared. Crutches and a knee scooter are relevant only if your technique means weeks off your foot — mainly a traditional open Lapidus, and to a lesser extent scarf or crescent osteotomy, per the table above.

The bigger picture: full recovery takes longer than the weight-bearing window

Weight-bearing clearance isn't the finish line. AAOS puts full recovery at up to 6 months, with follow-up visits sometimes needed for up to a year, and swelling common from 6 months to a year after surgery. Cleveland Clinic similarly notes bone healing generally takes 6-12 weeks, with a return to usual activity around 3 months and swelling that can last 6-9 months. HSS's own page states the recurrence rate — a bunion coming back — is about 20% over a patient's lifetime, an honest expectations number, not equipment-related.
